Please be advised that ASU will be closed on Thursday, November 25th and Friday November 26th for the Thanksgiving holiday. On Saturday, November 27th, we’ll be open 9:00am-6:00pm ET and will resume normal business hours on Monday, November 29th.

Humana Pharmacy Consent (54 agents out of 2000 have submitted consents. Humana can’t communicate with your members regarding the opportunity without the consent) don’t forget to get the consent signed at the time of application to help save your members co-pays on Tier 1 and Tier 2 drugs 90 day supply $0 copay. If you are using Ehub to do your applications it will be an option after signature on the application. There will be 3 questions. 1 PHI click no, 2 Humana pharmacy consent click yes, 3 MAF click no. Then click the grey button in the left column that says Auth & Sign answer the two questions then submit. If you are doing a paper application, the paper pharmacy consent that you can get signed is here.
Member Care Assessment Process
The member care assessment can be completed after every MA application and you can do the enrollment on any platform you choose (even paper) and complete the assessment within 5 days of the date on the application and earn a $50 admin fee. Survey Portal set up (called REVEL) is super easy and fast. And once you’re done, you can access the survey on: your Vantage portal on the Quote & Enroll card, direct on the REVEL portal, or in EHub.
