Nationally, 52% of Medicare beneficiaries* ages 65-74 manage three or more chronic conditions according to America’s Health Rankings 2023 Senior Report. To better support these consumers, UnitedHealthcare offers Chronic Condition Special Needs Plans (C-SNP) designed specifically for Medicare eligibles with diabetes, chronic heart failure, and/or cardiovascular disorders. These plans include benefits tailored to those with the qualifying conditions, some of which include:
UnitedHealthcare also offers some C-SNPs with benefits which are structured to be attractive to the full dual eligible Medicaid population; however, Medicaid is NOT required to enroll.
Members with qualifying chronic conditions may be eligible for a Special Enrollment Period (SEP) based on their qualifying condition and current plan. This is a great opportunity to grow your book of business during SEP!

Reminder: sales of these plans require the SNP certification. Watch for a new C-SNP training to join in March or contact your local market leader if you'd like to learn more. The UnitedHealthcare over-the-counter (OTC) benefit gives members enrolled in eligible Medicare Advantage, Dual Special Needs (D-SNP) and Chronic Special Needs (C-SNP) plans the power to purchase everyday health care items that support their well-being. Members can use their credit toward thousands of OTC products, with access to over 55 thousand retail locations. Additionally, for members enrolled in a D-SNP or select C-SNP plan, the benefit offers the unique opportunity to also use credits for purchasing healthy foods.
You can help members live healthier lifestyles by reminding them to take advantage of the benefits their Medicare Advantage plan may include. Don’t let important benefits like virtual medical care or mental health services get overlooked. Virtual medical care may provide more convenient access to providers for mild sicknesses, like the sniffles. Virtual mental health services may also help members get back to feeling like themselves or cope with challenging life circumstances.
