As with every year, UnitedHealthcare has reviewed the Medicare Plan Finder (MPF) on CMS.gov and found some areas where their Medicare Advantage (MA) plan benefits are not displayed correctly. The best source of information for UnitedHealthcare benefit and coverage details continues to be //uhc.com/medicare or the Summary of Benefits. However, it is understand that MPF is also an important tool for you and consumers. they have reached out to CMS and asked for the information to be corrected.

A UnitedHealthcare Chronic Special Needs Plan (C-SNP) gives you the opportunity to help individuals with certain chronic conditions find a plan made just for them. These plans have been expanded this year to reach 70% of eligibles and are designed to provide additional benefits that specifically support people who have diabetes, chronic heart failure, and/or cardiovascular disorders. Standard C-SNPs can be identified by “Complete Care” in the plan name.
With a C-SNP, UnitedHealthcare manages the verification process so all you need to do is complete the pre-assessment form and input the treating provider’s name and phone number as part of the application. Then UnitedHealthcare will reach out the provider to verify the member’s chronic condition. The provider listed does not have to be the member’s Primary Care Provider. They will take it from there.
